Job Responsibility1. Maintains personal training records
2. Assists the Supervisor in planning of work scheduling, patient care assignment
3. Any other duties as assigned by the Supervisor
4.Performs initial patient assessment to establish comprehensive information base on
patient physiological, emotional, cultural, psychological and spiritual needs
5. Identifies nursing problem, prioritizes and develops individualized nursing care plan
6. Performs ongoing patient assessment and revise plan of care based on patient's response
and condition
7. Maintains accurate and timely documentation of patient care and ongoing assessment of
patient's status
8. Provides direct and indirect patient care in a professional manner according to the
established procedures, policies and standards
9. Collaborate with other nurses and health team members by communicating information
and participating in team discussion to ensure continuity of care and effective
rehabilitation program.
10. Provides education to patients and families in a timely manner
11. Direct and supervise less skilled nursing or health care personnel
12. Maintains collaborative communication between patient/family and rehabilitation team
13. Adhere to infection-control policies and protocols
14. Demonstrates ability to handle emergency situations in a prompt, precise and professional
manner
